    Please complete this registration form for the NOAH Essentials of Arts in Health test. Your registration is not complete until you have also processed payment for taking the test.

    After you complete this online form and submit your payment for taking the test, you will receive a short email confirmation that your registration form has been received by the NOAH administration. Within one week, you will receive a more detailed message from a NOAH administrative staff member letting you know that your registration is complete or if something is missing. We will providing detailed instructions for taking the online test - including a list of dates and times to take it. Test registration is valid until May 1st, 2026. You must complete your test by May 1st, 2026.

    The test price is $275.00, and the final page of this form accepts a credit card payment.
