Personal Deliverance Ministry Form

A man with leprosy came to Him and begged Him on his knees. “If you are willing, you can make me clean. “Filled with compassion, Jesus reached His hand and touched the man, “I am willing.” “Be Clean!” Immediately the leprosy left him and he was cured. Mark 1:40-42 NIV

We believe the Lord is merciful and loves us. He wants to heal us in all areas of our lives. We can become “whole” persons in His kingdom. Wholeness is the heart of Jesus Christ.

As fellow believers, we will commit ourselves to seeking the Lord for your spiritual wholeness and healing.

Our sacrifice in the commitment is “time”, a precious and non-replaceable gift.” It is given freely in love;

A Time for Her Deliverance Team members do not discuss names, circumstances, or results with anyone outside the deliverance session unless written consent is given by the participant(s). We understand the need for discretion and to protect the names and times of those to whom we minister.

I, the undersigned, voluntarily request personal spiritual ministry for my life through the deliverance team and hereby agree to the following conditions: I understand that each of the sessions with A Time for Her Deliverance Team Members will be a time of personal ministry (spiritual guidance) based on the Bible, which is the Word of God.

I understand that no representation has been made either expressly or implied, that the team ministry is to be received as professional, psychological, or psychiatric therapy. I will diligently pray and seek God to show me His plan for my life, I will allow God to reveal my heart and the areas of my life He wants to change. I will allow myself to be open to growth and change. To the best of my ability, I will follow the assignment given to me by the A Time for Her Team.

I understand that the results of these sessions are solely within the power and wisdom of God. A Time for Her and team members cannot be considered to be responsible for any results, consequences, effects or lack thereof, which I may or may not experience as a result of these sessions.

I agree to inform A Time for Her of any additional participants or guests that plan to join me for the session. I understand that the deliverance team may find it beneficial and ask the person(s) to step outside the room in hopes of taking a deeper dive into the session.

I hereby agree to release and hold harmless, A Time for Her or the Deliverance Team participants from any liability resulting from this meeting and any subsequent meeting, dealing with this request for ministry. I am voluntarily seeking spiritual direction for my life through the Deliverance Team of A Time for Her.
