PASTORAL CONSENT

I acknowledge and understand that a separate pastoral consent form will be sent to my local pastor for him to approve my junior/camper's attendance at camp.

TERMS AND CONDITIONS

I, hereby, assume all of the risks of my child participating in any and all activities associated with Junior Camp, by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, or because of their possible liability without fault.

I certify that there are no health-related reasons or problems that preclude my child’s participation at Junior Camp.

In consideration of my child’s application and permitting them to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows

(A) I, do hereby, release and forever discharge the following entities: Apostolic Assembly of the Faith in Christ Jesus, Inc., the Arizona District and its affiliated pastors, churches, and all its properties, including the “Springs of Living Water Camp” in Prescott, AZ, AZJRMOP/ J4J Ministries and their staff liable or who might be claimed to be liable, whether or not herein names, none of whom admit any liability to the undersigned, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which I now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to the event at camp.

(B) I do hereby, indemnify, hold harmless, and promise not to sue the entities or persons mentioned in this paragraph for any and all liabilities or claims made as a result of my child’s participation in this activity, whether caused by the negligence of release or otherwise.

I acknowledge that The Apostolic Assembly of the Faith in Christ Jesus, Inc. and its directors, officers, volunteers, representatives, and agents are NOT responsible for the errors, omissions, acts, or failures to act, of any party or entity conducting a specific activity on its behalf.

If a serious emergency should occur, it may be necessary for a physician or staff nurse to attend to your child before camp staff can contact you or a designated physician. Such care can only be provided by agreeing to the following AUTHORIZATION FOR MEDICAL TREATMENT: I hereby consent to receive medical treatment, which may be deemed advisable in the event of injury, accident, and/or illness during this activity.
