Battle Boxing Gym Youth Camp
General Release of Liability and Participation Agreement
This General Release of Liability and Participation Agreement (“Agreement”) is entered into by and between Battle Boxing Gym (“Gym”) and the undersigned parent or legal guardian (“Parent/Guardian”) on behalf of the minor participant (“Participant”).
1. ACKNOWLEDGMENT OF RISK
I, the undersigned Parent/Guardian, understand that participation in boxing and related training activities involves inherent risks, including but not limited to physical injury, illness, permanent disability, or death. These risks may arise from participation in training exercises, use of equipment, contact with other participants, or conditions of the training environment.
I voluntarily assume all risks associated with my child’s participation in the Youth Boxing Camp.
2. CERTIFIED TRAINING AND SAFETY
Battle Boxing Gym affirms that all instruction will be conducted under the supervision of certified boxing trainers, including Coach Elijah and Coach Aidan, who are responsible for implementing proper training techniques and maintaining reasonable safety standards.
I understand that while all reasonable precautions and safety measures will be taken, no activity is completely risk-free.
3. MEDICAL AUTHORIZATION
In the event of an injury or medical emergency, I authorize Battle Boxing Gym staff to seek appropriate medical treatment for my child. I agree to be responsible for any medical expenses incurred.
4. RELEASE OF LIABILITY
To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Battle Boxing Gym, its owners, employees, contractors, coaches (including Coach Elijah Seawright and Coach Aidan Lazenby), and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Participant while participating in camp activities.
5. PERSONAL PROPERTY
Battle Boxing Gym is not responsible for any personal items that are lost, stolen, or damaged during the camp. Participants are encouraged to leave valuables at home.
6. CODE OF CONDUCT AND DISMISSAL POLICY
Participants are expected to conduct themselves in a respectful and disciplined manner at all times.
Battle Boxing Gym reserves the right to dismiss any participant from the camp for inappropriate behavior, including but not limited to:
- Disrespect toward coaches or other participants
Disruptive or unsafe conduct
Failure to follow instructions or camp rules
Dismissal due to behavioral issues will result in immediate removal from the camp without refund of any fees paid.
7. PHOTO/VIDEO RELEASE
I grant Battle Boxing Gym permission to use photographs or videos of my child taken during camp for promotional and marketing purposes.
8. AGREEMENT AND SIGNATURE
By signing below, I acknowledge that I have read, understood, and agree to the terms of this Agreement. I affirm that I am the legal parent or guardian of the Participant and have the authority to sign on their behalf.
