I hereby give my permission for my child to participate in all activities organized by Trench Mob USA during the selected camp session. I understand and acknowledge that participation in these activities involves certain inherent risks and potential hazards.

I agree to release, hold harmless, and indemnify Trench Mob USA, its staff, coaches, volunteers, affiliates, event partners, sponsors, and the owners or operators of any facilities used, from any and all liability for injuries, losses, or damages that may occur as a result of my child's participation.
