I consent to receiving lash extensions services from Lashed by Bey and acknowledge that I have had the opportunity to ask questions before my appointment
I release Lashed by Bey from liability related to complication resulting from failure to follow the aftercare instructions, undisclosed medical condition and allergies
I understand that I should inform my lash artist immediately if I experience discomfort during the appointment
I understand that if I arrived with lash extensions applied by another lash artist, additional charge may apply for removal and corrective work
