I, as parent/guardian of the above participants, grant permission for these participants to travel to St. Vincent de Paul at 3001 East 30th Street on April 26, 2025.

I hereby release and save harmless Bishop Chatard High School/St. Vincent de Paul and any and all of its employees from any and all liability for any and all harm arising as a result of this trip. In the event of an emergency, I understand that school authorities will make every effort to contact me, but I authorize those authorities to seek emergency medical treatment in a life-threatening or serious situation.

I further understand that all school rules and regulations apply to the conduct of any school-sponsored activity away from the premises.
