Recognizing the possibility of injury associated with participation in any event, and in consideration of Bishop Chatard High School (BCHS) and its affiliates accepting the registrant for these activities, I hereby release, discharge, and indemnify BCHS, its staff, sponsors, employees, and associated personnel, including the Archdiocese of Indianapolis, from any claims arising from the registrant’s participation.

I also authorize BCHS personnel to act in their best judgment in any situation requiring medical attention.

I give permission for BCHS to use photographs of my child taken during camp activities for marketing or promotional purposes.
