I, as parent/guardian of the above child, grant permission for this child to travel by BCHS Bus to the Youth Rally for Life at the Indiana Convention Center on January 22, 2026.

I hereby release and save harmless Bishop Chatard High School and any and all of its employees from any and all liability for any and all harm arising to as a result of this trip. In the event of an emergency, I understand that school authorities will make every effort to contact me, but I authorize those authorities to seek emergency medical treatment in a life-threatening or serious situation.

I further understand that all school rules and regulations apply to the conduct of any school-sponsored activity away from the premises.
