PARTICIPATION WAIVER & RELEASE OF LIABILITY 1. ASSUMPTION OF RISK I understand that participation in basketball activities, including but not limited to practices, games, tournaments, training sessions, and related events associated with Next Level 3 (NL3) involves inherent risks. These risks include, but are not limited to:Physical injury/Sprains, fractures, or concussions,Serious bodily injury/Illness or exposure to communicable diseases Property damage. I voluntarily assume all risks associated with participation in NL3 activities. 2. RELEASE AND WAIVER OF LIABILITY! In consideration of being allowed to participate in Next Level 3, I, on behalf of myself and/or my minor child, hereby release, waive, discharge, and hold harmless:Next Level 3 (NL3)Benson Elite Training Academy,Chillicothe City Schools Facility owners and operators League directors, coaches, referees, staff, volunteers Sponsors and affiliated organizations. From any and all claims, demands, actions, or causes of action arising out of or related to any injury, illness, loss, or damage sustained while participating in NL3 activities, whether caused by negligence or otherwise, to the fullest extent permitted by law. 3. MEDICAL AUTHORIZATIONI certify that the participant is physically able to participate in athletic activities. In the event of injury or emergency, I authorize NL3 staff to seek medical treatment as deemed necessary. I understand that I am responsible for any medical expenses incurred. 4. CODE OF CONDUCTI agree that the participant and family members will conduct themselves in a respectful and sportsmanlike manner. Failure to comply may result in removal from league activities without refund. 5. PHOTO & MEDIA RELEASEI grant permission to Next Level 3 and Benson Elite Training Academy to use photographs and/or video recordings of the participant for promotional, marketing, or social media purposes without compensation. 6. INDEMNIFICATIONI agree to indemnify and hold harmless Next Level 3, Benson Elite Training Academy, Chillicothe City Schools, and all associated parties from any claims brought by or on behalf of the participant arising from participation in NL3 activities.7. ACKNOWLEDGMENT. I have read this waiver carefully and understand its terms. I understand that by signing this document, I am giving up substantial legal rights.I sign this voluntarily and without inducement.
