PLEASE READ CAREFULLY! This agreement includes a release of liability and waiver of legal rights and deprives you of the right to sue Bowlin Farms Doggie Adventures LTD., and related parties. Do not sign this agreement unless you have read it in its entirety. Please seek the advice of legal counsel if you are unsure of its effects. This waiver of liability, assumption of risk and indemnification agreement for services related to Daycare/Day school program.
LEGAL DOG OWNER: As the owner, you acknowledge and understand that you are the legal owner or agent of the dog described in the correlating application form above.
PET HEALTH: As the owner, you affirm that my dog is in good health and is currently on all required vaccinations listed on page 1 of the information sheet. (Distemper/DHLPP, Rabies, Bordetella, Parvo) My dog is free of any fleas, ticks and lice and has not been ill with any known contagious viruses in the last 30 days. I understand that while my dog is fully vaccinated, that vaccines are not guaranteed to keep my dog free of any sickness and there is a small risk that my dog may contract a contagious disease or illness. I agree that should this occur; I am responsible for my own pet’s care and medical attention.
DUTY TO DISCLOSE: I agree to disclose any and all previous or current medical or behavior issues or concerns of my dog so that Bowlin Farm Doggie Adventures Ltd.’s staff can make a determination of the suitability for the Daycare program. I agree to disclose if my female dog is showing signs of being in heat and will not bring my dog into the Daycare program. If any information is withheld, especially related to behavior issues or my dogs heat cycle, and it is found out later while your dog is in Daycare program, Bowlin Farm Doggie Adventures has the right to terminate your dog from the Daycare program.
RIGHT TO REMOVE: I understand that Bowlin Farm Doggie Adventures reserves the right to permanently remove/refuse a dog from its Daycare program at any time.
INCIDENTS: I understand that although all dogs are fully supervised, incidents or injuries may occur from playing or just being out in the Daycare area, which include, but not limited to, escapes, bites, scrapes, scratches, sprains or other minor injuries. All employees or agents have animal first aid certificates where they are trained to their best abilities to treat your dog with the appropriate care at that time. You as the owner will always be notified to the best of our employees’ or agents ability if an event ever happens that your dog needs medical attention. (this is where updating all contact information with us is very important) If your dog is in need of medical attention, and the employees are not able to get ahold of you as the owner or your emergency contact. The employees will attend Bowlin Farms Doggies Adventures designated Veterinary Clinic at Cobequid animal hospital (15 Cobequid rd., Lower Sackville, NS / 902-865-8110) for your dog to get the proper care they may need in a timely manner. In an event that your dog is not the one that is in need of medical attention and is found to be the aggressor. We will be contacting you the owner advising that all medical bills associated with the injuries will be your responsibility to cover. (this includes but is not limited to follow up medical attention if needed)
PAYMENTS: I understand that ALL services provided, and packages sold are non-refundable. Packages can be transferred to only one dog currently enrolled at the facility. I agree to pay the same day (before 8PM) for my dog’s visit to the facility. All payments are to be made by cash, cheque or email transfer. Email transfers are all to be sent to bfdadventures@gmail.com. Email transfer passwords are to be either your “dogs name” or “bowlinfarms”. If you are changing the password at any time, I agree to let BFDA staff be aware as soon as possible.
