Boys & Girls Club Membership Form
To complete the registration process and secure your child's spot in our summer program, you will also need to submit an official BGCA membership form if their membership is not current within 2025.

The $15 annual membership fee is due by the first day of camp, along with the first week of tuition for campers at the Sixth Street and Turner Street Clubhouses.

There are no summer tuition fees for the Teen Center or residents of Cumberland Gardens.

How will you submit your child's BGCA membership form?*
Before uploading the form, please check to make sure:
1. All fields are fully completed and easy to read.
2. The document is scanned as a PDF file (not just a photo).
3. The front/back of the form are together in one PDF file.
4. The PDF quality is high enough to read and print.
If you do submit the form online, please also bring the original printed copy with your child on the first day of camp.
