50/50 Approval Request and Supply Order Form

50/50 Raffle Ticket spools must be purchased through Big E. Each TEAM is permitted to sell tickets at their designated price for 50/50 drawings, but we recommend a 1/$1, 3/$2, 10/$5 is a common threshold or starting point. We intend on orders being able to be picked up at the HS. Although likely filled within 3 days, please permit a week from order date. Questions, email bigesgc167@gmail.com.

Provide any dates that your TEAM is requesting to conduct 50/50 drawings. Include the anticipated location these will be held.
It is recommended that all dates you anticipate conducting 50/50 drawings be requested at the same time; however, you can specify certain dates and/or events, and then complete another form for additional dates. All dates do need to be requested and approved. Location game will be conducted should also be provided.

Petty Cash Requested $
A check in the amount indicated will be issued to the name input above for purposes of cashing for small bills that you will retain in your 50/50 zipper bag. IMPORTANT NOTE: It is recommended you place a highly visible notecard in your bag to remind assigned persons responsible for your 50/50 to subtract petty cash amount before verifying sales funds and announcing winner so your TEAM does not give those funds away.

I authorize the transfer of $10.00 per each single item selected above from our TEAM account to Big E. I also authorize a check request from our TEAM account for any Petty Cash amount indicated above for purposes of retaining to use for drawings.
