USA Lacrosse Member:
Prior to registering for any Bitter Lax Academy program, players must be a USA Lacrosse member with an active account. Players who are USA Lacrosse members, are covered by the USA Lacrosse Insurance Program for the term of their membership. Please visit the USA Lacrosse website to learn more.

Registration and Refunds:
Registration and payment for the Billy Bitter Lacrosse Academy must be completed and submitted online. Full payment is required at the time of registration to reserve the child’s space, unless a payment plan has been agreed upon prior to the start of an event. Due to limited space, refunds will not be given once an event start date has passed. There will be no proration of fees for any days a child participant cannot attend.

Medical Needs/Allergies:
Billy is not permitted to administer medication to program participants. There is no certified athletic trainer on-site during the academy sessions. In the event of a medical emergency, Billy will call 911 and notify the parents. All known allergies and forms of medications must be emailed to Billy prior to registering your child.

What to Wear:
Children should dress appropriately for what they would wear normally for a sports related event. All academy sessions will take place outside. Clothing that displays drugs, alcohol, tobacco, offensive language, excessive bagginess, or is excessively revealing will not be permitted.

Personal Belongings:
Please put the child’s name on all articles of clothing, lacrosse gear, snack bags, bags, etc. Children should not bring toys, mobile phones, electronic devices, jewelry, money, or any possession of value with them to any of the programs. Children will be responsible for their belongings.

Sunscreen/Insect Repellent:
Please apply sunscreen and insect repellent prior to the start of each day your child attends a session. Children may bring sunscreen and insect repellent but must be able to reapply on their own.

Nutrition/Snacks:
Players should bring a prefilled water bottle.

Inclement Weather:
If we are unable to hold a session due to inclement weather, Billy will coordinate and schedule a make-up session. No more than two make-up sessions will be given.

Behavior Management/Discipline Policy:
Billy will create a fun and safe environment for players in the program. Praise and positive reinforcement are used as effective methods of behavior management. Children who do not respond to these methods or who are destructive to others or to property will be dealt with in a professional, positive, and timely manner to correct the behavior.

NOTE: Immediate dismissal from the program can occur at any time given severe circumstances as determined in Billy's sole discretion. Refunds for missed days due to a discipline dismissal will not be granted.

Informed Consent, Waiver and Release of Claims:
By signing this form, I, as parent/guardian, permit the Billy Bitter Lacrosse Academy to use pictures of my child(ren) as a program participant in Billy Bitter Lacrosse Academy promotions including without limitation promotional literature, videos, social media and the Billy Bitter Lacrosse website. I understand my child(ren)’s name(s) will not be published.

I, as parent/guardian of ____________ (“Child”) understand the risks and dangers of the sport of lacrosse and Billy Bitter Lacrosse Academy activities and hereby assume all inherent risks and dangers related to and arising from Child’s participation in Billy Bitter Lacrosse Academy activities. Child is fit for the program(s) in which I have enrolled Child. In consideration for Bitter Lax Academy allowing Child to participate in Bitter Lax Academy activities, I HEREBY AGREE TO WAIVE ANY AND ALL CLAIMS AGAINST AND TO HOLD HARMLESS, RELEASE, INDEMNIFY AND AGREE NOT TO SUE BITTER LAX ACADEMY AND ITS AFFILIATED AND RELATED ENTITIES AND PERSONS INCLUDING WITHOUT LIMITATION VOLUNTEERS, EMPLOYEES, AND AGENTS (COLLECTIVELY, THE “RELEASEES”). I HEREBY WAIVE AND RELEASE EVERY CLAIM AND ANY LIABILITY THAT I OR MY CHILD MAY ALLEGE AGAINST RELEASEES (INCLUDING WITHOUT LIMITATION ATTORNEYS’ FEES AND COSTS) AS A DIRECT OR INDIRECT RESULT OF INJURY, INCLUDING DEATH, LOSS, PROPERTY DAMAGE OR EXPENSE, WHICH I OR MY CHILD MAY SUFFER ARISING IN WHOLE OR IN PART OUT OF MY CHILD’S PARTICIPATION IN BITTER LAX ACADEMY ACIVITIES, INCLUDING BUT NOT LIMITED TO THOSE CLAIMS BASED ON ANY RELEASEE’S ACTION, INACTION, OR ALLEGED OR ACTUAL NEGLIGENCE.I ACKNOWLEDGE THAT, BY SIGNING THIS DOCUMENT, I AM RELEASING THE RELEASEES FROM ALL CLAIMS AND LIABILITY, AND THAT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS. IF A COURT OF COMPETENT JURISDICTION DETERMINES THAT ANY PART OR PROVISION OF THIS RELEASE FORM IS UNENFORCEABLE, SUCH UNENFORCEABLE PART OR PROVISION SHALL BE SEVERABLE AND THE REMAINER OF THIS RELEASE FORM SHALL BE ENFORCEABLE. THISSIGNUPAND RELEASE FORM APPLIES TO ALL ACTIVITIES IN WHICHMY CHILD ENGAGES DURING BITTER LAX ACADEMY ACTIVITIES, REGARDLESS OF WHETHER SUCH ACTIVITY IS A PART OF A FORMAL PROGRAM.

I HAVE READ THIS RELEASE CAREFULLY BEFORE SIGNING. I AM VOLUNTARILY AND KNOWINGLY SIGNING THIS RELEASE.

I understand that no insurance coverage for players is provided by Billy Bitter Lacrosse. All insurance coverage is handled through the governing body of the sport, USA Lacrosse.
