USA Lacrosse Member:
Prior to registering for any Bitter Lacrosse program, players must be a USA Lacrosse member with an active account. Players who are USA Lacrosse members, are covered by the USA Lacrosse Insurance Program for the term of their membership. Please visit the USA Lacrosse website to learn more.
No Cell Phone Policy:
Bitter Lacrosse prohibits any use of cell phones and/or any other electronic devices (apple watch, fitbit, ipad, laptop, xbox etc). All cell phones and electronic devices should be left at home. In case of emergency, parents can reach their child at this number; (843) 310-3372 by text. If a camper must speak to their parents while at camp they will be granted that option upon request.
Health and Medical Needs/Allergies:
For Bitter Lacrosse camps at least one certified athletic trainer will be present throughout each week of camp. In the event of a medical emergency, the athletic trainer will administer first aid, CPR, and rescue in the best interest of the child. Parents will be contacted if care is administered. Allergy medicines, inhalers, Epi-Pens, and other forms of medications will be administered by the athletic trainer. All other over-the-counter drugs (Tylenol, Advil, etc.) may be administered by the athletic trainer. Overnight campers may not have any over-the-counter or prescription drugs in their rooms.
Meals:
For overnight campers, Bitter Lacrosse will provide a healthy breakfast, morning snack, lunch, afternoon snack, and dinner. For day campers, Bitter Lacrosse will provide a healthy morning snack, lunch, and afternoon snack. Water will always be available for campers. Bitter Lacrosse is mindful of a child's allergy -- please specify if your child has any allergies in the section above. Special accommodations will be made for those with an allergy to food or drink.
Behavior Management/Discipline Policy:
The Bitter Lacrosse staff will create a fun and safe environment for players in the program. Praise and positive reinforcement are used as effective methods of behavior management. Children who do not respond to these methods or who are destructive to others or to the property will be dealt with in a professional, positive, and timely manner to correct the behavior. All parents will receive the Bitter Lacrosse Camp Code of Conduct prior to camp for review.
NOTE: Immediate dismissal from the program can occur at any time given severe circumstances as determined in Bitter Lacrosse's sole discretion. Refunds for missed days due to a discipline dismissal will not be granted.
Personal Belongings:
Please put the child’s name on all articles of clothing, lacrosse gear, bags, etc. Children should not bring toys, mobile phones, electronic devices, jewelry, or any possession of value with them to any of the programs. Campers will be responsible for their belongings.
Nutrition/Snacks:
Campers should bring healthy snacks and drinks to camp. Water bottles are recommended. Water bottles should be clearly labeled with the child’s name. Sodas are not recommended. Please do not send child with gum or candy.
Informed Consent, Waiver and Release of Claims:
By signing this form, I, as parent/guardian, permit the Bitter Lacrosse to use pictures of my child(ren) as a program participant in Bitter Lacrosse promotions including without limitation promotional literature, videos, social media and the Bitter Lacrosse website. I understand my child(ren)’s name(s) will not be published.
I, as parent/guardian of _________ (“Child”) understand the risks and dangers of the sport of lacrosse and Bitter Lacrosse activities and hereby assume all inherent risks and dangers related to and arising from Child’s participation in Bitter Lacrosse activities. Child is fit for the program(s) in which I have enrolled Child. In consideration for Bitter Lacrosse allowing Child to participate in Bitter Lacrosse activities, I HEREBY AGREE TO WAIVE ANY AND ALL CLAIMS AGAINST AND TO HOLD HARMLESS, RELEASE, INDEMNIFY AND AGREE NOT TO SUE BITTER LACROSSE AND ITS AFFILIATED AND RELATED ENTITIES AND PERSONS INCLUDING WITHOUT LIMITATION VOLUNTEERS, EMPLOYEES, AND AGENTS (COLLECTIVELY, THE “RELEASEES”). I HEREBY WAIVE AND RELEASE EVERY CLAIM AND ANY LIABILITY THAT I OR MY CHILD MAY ALLEGE AGAINST RELEASEES (INCLUDING WITHOUT LIMITATION ATTORNEYS’ FEES AND COSTS) AS A DIRECT OR INDIRECT RESULT OF INJURY, INCLUDING DEATH, LOSS, PROPERTY DAMAGE OR EXPENSE, WHICH I OR MY CHILD MAY SUFFER ARISING IN WHOLE OR IN PART OUT OF MY CHILD’S PARTICIPATION IN BITTER LACROSSE ACIVITIES, INCLUDING BUT NOT LIMITED TO THOSE CLAIMS BASED ON ANY RELEASEE’S ACTION, INACTION, OR ALLEGED OR ACTUAL NEGLIGENCE.I ACKNOWLEDGE THAT, BY SIGNING THIS DOCUMENT, I AM RELEASING THE RELEASEES FROM ALL CLAIMS AND LIABILITY, AND THAT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS. IF A COURT OF COMPETENT JURISDICTION DETERMINES THAT ANY PART OR PROVISION OF THIS RELEASE FORM IS UNENFORCEABLE, SUCH UNENFORCEABLE PART OR PROVISION SHALL BE SEVERABLE AND THE REMAINER OF THIS RELEASE FORM SHALL BE ENFORCEABLE. THISSIGNUPAND RELEASE FORM APPLIES TO ALL ACTIVITIES IN WHICHMY CHILD ENGAGES DURING BITTER LACROSSE ACTIVITIES, REGARDLESS OF WHETHER SUCH ACTIVITY IS A PART OF A FORMAL PROGRAM.
I HAVE READ THIS RELEASE CAREFULLY BEFORE SIGNING. I AM VOLUNTARILY AND KNOWINGLY SIGNING THIS RELEASE.
I understand that no insurance coverage for players is provided by Bitter Lacrosse. All insurance coverage is handled through the governing body of the sport, USA Lacrosse.
