  • By submitting this form and enrolling in a coaching program with TransformE Online Training LLC, you acknowledge and agree to the following terms:
    1. Medical Disclaimer
    I understand that the fitness and nutrition coaching provided by TransformE Online Training LLC is for educational and informational purposes only.
    This is not medical advice, and I should consult a physician or healthcare professional before beginning any fitness or nutrition program.
    I confirm that I have no medical conditions or injuries that would prevent me from safely participating. If I do, I have obtained clearance from my physician.
    2. Assumption of Risk & Liability Waiver
    I voluntarily assume all risks associated with exercise, including but not limited to injury, illness, or death.
    I release and discharge Brett Araniti and his employees, contractors, and affiliates from any liability resulting from my participation.
    I acknowledge that results are dependent on my effort and adherence and are not guaranteed.
    3. Refund & Payment Policy
    All payments are non-refundable once services have been rendered.
    Subscriptions and coaching services must be canceled at least 30 days before renewal to avoid further charges.
    I understand that failure to complete my program or adhere to recommendations does not entitle me to a refund.
    4. Client Responsibilities
    I agree to follow the prescribed fitness and nutrition plan and communicate honestly and regularly with my coach.
    I understand that my progress depends on my commitment, and my coach is not responsible for a lack of results due to non-adherence.
    5. Confidentiality & Data Use
    Any personal data collected will be kept confidential and used solely for program design.
    TransformE Online Training LLC will not share or sell my personal information to third parties.
    6. Code of Conduct & Termination Clause
    I agree to maintain respectful communication with my coach.
    TransformE Online Training LLC reserves the right to terminate services if I engage in harassment, misconduct, or non-compliance with program guidelines.
    If terminated due to harassment, misconduct, or non-compliance, no refund will be issued.
    7. Governing Law & Dispute Resolution
    This agreement shall be governed by the laws of Connecticut/United States.
    Any disputes shall be resolved through mediation/arbitration first before any legal action.
