In order to help streamline the booking process, please begin by filling out any relevant information I may need for your appointment below. If there is a question you do not understand or are not sure how to answer, give a best guess and let me know at the end how I can help. Once this has been filled out you will be automatically put onto a waitlist. I check requests weekly. Upon scheduling the appointment, a $50 non-refundable/non-transferable deposit must be paid within 72 hours via Venmo (@brock_tattoos) or cash. I'm grateful you chose me for your tattoo artist, excited to work on your request, and I appreciate your patience!
