Female: Black leotard (no skirts or jewels), pink tights, pink slippers, hair should be tied back and off face, long hair must be worn in a bun.

Male: White t-shirt, plain sweatpants, black slippers
I will provide my own dancer's uniformI would like to purchase the uniform now and have my child fitted at the dance space on the day of their first class (Uniform will be provide at the second class)

Students ages 3+ must follow the school dress code stated above.
Registration is non-refundable.
There will be no makeups for missed classes.
By completing this form, parent gives consent for their child to be photographed and filmed for publicity purposes.
I hereby release Allegra Glinsky and Class Space from all responsibilities for any injuries sustained by my child while on class premises before, during, or after dance class.
