2026 Homeschool Prom Guest Waiver

  What is included: Dinner catered by Olive Garden and professional DJ services with dancing.
  - Please read and fill out this entire form, including all required information, and provide a signature for the attending guest and a parent/guardian’s signature for each minor attending.
  - This guest registration form must be completed and submitted by the guest's parent to complete the ticket reservation.
  - By signing below, I confirm that I have read, understand, and will comply with the Homeschool Prom Code of Conduct and Attire document provided with this form. I understand that any non-compliance may result in not being admitted or being asked to leave the Prom. I also agree to ensure that any guests I bring will comply with the Homeschool Code of Conduct and attire. I also agree that my picture may be taken and used for promotional purposes.
  Liability Disclaimer: I understand and agree that the sponsoring Homeschool Prom Committee and the Rich and Snyder Family, will not be held responsible and/or liable for any injuries, losses, or damages (bodily, personal, emotional, property, accidental, or otherwise) incurred during this event including at the entrances, exits, and surrounding areas, regardless of whether such injury or damage results from negligence of Homeschool Prom Committee and the Snyder or Rich families, or otherwise.

- Guest Initial* Attendees need to behave in a respectful, mature manner.
- Guest Initial* Attendees must abstain from improper male/female physical contact, gossip, slander, profanity, drinking, vaping, smoking, and any behavior that draws undue attention to themselves.

  Males (assigned at birth): Dress shirt and tie at minimum with dress pants. No jeans (of any color).
  Females (assigned at birth): Formal Gowns, but please dress modestly. Dresses should be no more than 5 inches above the knee, including while dancing. Clothing may not be sheer, too tight, or form-fitting. No plunging necklines permitted.
