Junior Skating	members who participate in synchronized ice skating and are aged under 18 years includes a Non-Skating membership for the applicant's parent/guardian	$95
Aussie Skate	members who participate in synchronized ice skating in an Aussie Skate (Skate School) team	$35
Non-Skating	members and coaches who do not participate in synchronized ice skating, are aged 18 years or over and support the objectives of the Club	$35

Compliance

I agree to be bound by the Rules and By-laws of the Club and to comply with the Club's Behaviour Management Policy.

Risk

I understand that participation in Club activities involves the risk of injury and/or loss or damage to my property. I accept that I participate in Club activities at my own risk.

I accept that the Club and its management, coaches, officers and agents are not liable for any personal injury, loss or damage of property or expenses, including medical expenses, which I might suffer as a result of involvement in any
Club activity.

I am aware of insurance cover held by the Club and by Ice Skating Australia.

Media Consent (Optional)

The Club may take photographs and/or video recordings of members during Club activities for purposes such as training, education, publicity and promotion. The Club will comply with Ice Skating Queensland policies regarding images, including policies relating to children.

You may choose whether or not to consent to the use of your image by selecting the appropriate option below. Your decision will be respected and will not affect your membership or participation in Club activities.

If you provide consent, you agree that the Club may use appropriate images of you for Club purposes and release the Club from any liability arising from such use.

Medical Treatment

I authorise the Club to arrange all necessary medical treatment, including ambulance transport, which might be required by me while involved in any Club activity. I acknowledge that the costs of any such treatment will be my responsibility
