Medical Clearance & Responsibility
I understand that Post-Operative Lymphatic Drainage and Sculpting services may only be performed after I have received medical clearance from my surgeon or physician.
If I am completing this form prior to surgery, I acknowledge that I will not receive services until I am medically cleared and it is my responsibility to confirm when I am approved to begin treatment.
If I have already had surgery, I confirm that I have received clearance and have disclosed my surgery details, medications, complications, and relevant medical history.
I understand these services are non-medical supportive services and do not replace medical care. I accept full responsibility for my medical readiness and disclosures.
Risks & Results Disclaimer
I understand that post-operative services may cause temporary swelling, bruising, tenderness, soreness, and fluid movement.
I acknowledge that healing timelines vary and results depend on my body’s healing response and compliance with aftercare instructions, including garment use, hydration, diet, and activity level.
No specific results are guaranteed.
Mobile Arrival Window & Travel Policy
Services are mobile and require travel and setup time, appointment times are confirmed the day prior.
Clients must reserve a minimum 2-hour arrival window to allow for traffic, prior appointments, weather, and unforeseen delays. Exact arrival times cannot be guaranteed.
Delays due to reasonable travel circumstances do not qualify for cancellation or refund. Clients must remain available during the entire arrival window.
By booking, I acknowledge and agree to this arrival policy.
Arrival Policy:
If the therapist arrives at the scheduled location and the client is unavailable, unresponsive, refuses service, or is not ready within 15 minutes, one full session will be deducted from the package.
If booked as a single session, 100% of the service fee will be charged.
Because travel time, preparation, and scheduling are reserved exclusively for each client, forfeited sessions serve as the agreed cancellation fee.
Forfeited sessions cannot be reinstated, credited, transferred, or refunded.
Repeated late cancellations may result in refusal of future services.
Mobile Service Cancellation & Package Forfeiture Policy
Services are mobile and travel time is reserved specifically for each client, the following applies:
A minimum of 24-hour notice is required to cancel or reschedule.
Cancellations made with less than 24 hours notice will result in forfeiture of one session from the package.
If booked as a single prepaid session, 50% of the total service fee will be retained as the cancellation fee. The remaining balance will be applied as a credit toward a rescheduled appointment.
Professional Conduct
I understand this is a professional therapeutic service. Any inappropriate behavior, comments, or gestures will result in immediate termination of the session without refund.
Media & Recording Policy
To protect client privacy, professional boundaries, and proprietary techniques, audio or video recording of services is strictly prohibited without prior written consent.
If recording is discovered at any time during the session, services will be immediately terminated and full payment will be charged. No refunds will be issued.
Package Payment & Commitment Policy
By reserving a service package, the client agrees to the following terms:
A 50% deposit is required at the time of booking to secure the package.
The remaining 50% balance must be paid after the first session.
Packages cannot be changed to individual sessions or switched to a different package once reserved.
Package sessions may be used in the way that best fits the client’s schedule within the recommended timeframe.
No Refund Policy
All services, deposits, and packages are non-refundable and non-transferable. Unused sessions must be used within the designated timeframe.
Results vary based on individual healing, compliance, and lifestyle factors. Dissatisfaction with results does not qualify for a refund.
Medical Rescheduling Exception
If pregnancy or a verified medical complication delays surgery, prepaid post-operative services may be held as account credit.
Credits may be used once surgery is rescheduled and medical clearance is obtained. Medical documentation may be required.
Credits are valid for up to 12 months from the original purchase date unless otherwise approved in writing. No refunds will be issued; payments remain as account credit only.
