Dog Adoption Application

Thank you for coming to CAAWS to look for your new companion. Our goal is to adopt companion pets to permanent, responsible, loving homes and to help you find the right dog for your household. It is important for all concerned that the best placement possible be made—the right dog in the right home. Please take the time to fill out this form completely. The information you provide will help us share our experiences with you and other suggestions for proper pet care as well as advising you on the responsibilities of pet ownership.

Even though you are filling this out online, we must still see you in person. Please bring all children and dogs in the home with you at that time so that the interaction can be seen between all parties. This application is not a guarantee that this animal will be yours, this is only the first step in the application process.

When we close to the public at 4pm, our adoption committee will meet and discuss the submitted applications and whether or not each is a viable candidate for that dog and its particular needs. Each applicant will be contacted regardless, but if they are approved a date and time for a home check will be set up after a vet check and apartment’s pet policy is checked if needed. An adoption counselor will bring the dog to the adoptor’s home and if all is well the adoption contract will be completed and the dog will be a permanent part of your family!

We would like you to be aware that our adoptions are completed on a "best fit" basis, rather than "first come." This means that we will accept multiple applications on one dog, but it also means that no matter when you arrive between 12pm and 4pm, you will have an opportunity to visit with all of our available dogs.

Before applying for adoption, please read the following carefully. In order to qualify to adopt a pet from CAAWS, you MUST:

Be 21 years of age or older;
Have and show a current ID showing your age and present address;
Be willing and able to provide proper training, medical and stable lifelong care for the entire life of the pet;
Pay an adoption fee (NON-REFUNDABLE); and
UNDERSTAND THAT CAAWS RESERVES THE RIGHT TO DENY ANY APPLICATION FOR ADOPTION.
