I, First Name Last Name , declare myself as an applicant for student government at West Hills Middle School for the 2021-2022 school year. I understand that, if accepted, I will be enrolled in student government as an elective class. I understand that I have to be enrolled in this class in order to run for election as a Student Body Officer at the beginning of next school year. I am aware of the responsibilities and time commitments that I will have as a member of WHMS student government. I am aware that as a part of student government, I will serve as an example to other students and will conduct myself accordingly. I understand that I am expected to complete all my responsibilities in student government while maintaining good grades and outstanding citizenship. I am aware that if I fail to meet the minimum grade standard of 3.0 and no Ns or Us for any of the four quarters, I will be placed on probation. I understand that after the second infraction due to grades or behavior, I will be removed from office. I know that my signature indicates that I am willing and ready to make the needed commitment to follow all school and district policies. If I do not adhere to these responsibilities and policies, I will be removed from office and/or receive other disciplinary action. Candidate Initials I have completed the entire application.
