Summer Camp Disclaimer – Cat Depot
By enrolling your child in the Cat Depot Summer Camp program, you acknowledge and agree to the following:

Assumption of Risk
Participation in activities involving animals carries inherent risks, including but not limited to scratches, bites, allergic reactions, or other unforeseen incidents. While Cat Depot takes reasonable precautions to ensure safety, we cannot guarantee that injuries or incidents will not occur.

Health & Medical Consent
You certify that your child is in good health and able to participate in camp activities. You authorize Cat Depot staff to seek emergency medical treatment for your child if necessary, and you agree to be responsible for any associated costs.

Allergies & Medical Conditions
You must disclose any known allergies (especially to animals), medical conditions, or behavioral concerns prior to participation. Cat Depot is not responsible for complications arising from undisclosed conditions.

Animal Interaction & Risk Acknowledgment
While Cat Depot takes every precaution to ensure safe and positive interactions, working with animals always carries some inherent risk. Campers may experience minor scratches or nips, even when following proper handling techniques. All interactions with cats are closely supervised, and campers are taught appropriate, gentle handling and how to recognize feline body language.

By participating in camp activities, parents/guardians acknowledge and accept these risks. Any incidents will be addressed immediately by staff, and parents/guardians will be notified as appropriate.

Behavior Expectations
Cat Depot reserves the right to dismiss any child whose behavior is deemed unsafe, disruptive, or harmful to animals, staff, or other campers, without refund.

Photo & Media Release
By registering, you grant permission for your child to be photographed during camp activities. Images may be used for educational and promotional purposes.

Personal Belongings
Cat Depot is not responsible for lost, stolen, or damaged personal items brought to camp.

Refund/Cancelation Policy
Requests for refunds must be received in writing. An 50% refund will be given if cancellation is received five (5) days or more before the first day of camp. NO REFUNDS ARE GIVEN FOR CANCELLATIONS ON OR AFTER THE FIRST DAY OF CAMP. Cat Depot reserves the right to limit program enrollment and to cancel program completely if registration is below the required minimum.
PERMISSION FORM

I understand that all possible precautions are taken to ensure that the programs and activities at the Cat Depot, Inc. camp (“Camp”) are conducted in a safe and responsible manner. However, I further understand that because of the nature of Camp activities, and susceptibility to natural hazards, regardless of the high degree of supervision, there is a potential for accidental injury or death during any Camp activity. As a parent and/or legal guardian, I permit my minor child(ren) to engage in all Camp activities, including but not limited to arts, crafts, indoor games, interacting with cats and kittens. I do recognize these risks and agree to allow my child(ren) to participate in all activities offered in the Camp program in which I am enrolling my child(ren). I AGREE ON BEHALF OF MY CHILD(REN) AND MYSELF TO ASSUME THE RISKS ASSOCIATED WITH ALL ACTIVITIES WHICH OCCUR AT THE CAMP. I HEREBY RELEASE, INDEMNIFY AND AGREE TO HOLD HARMLESS CAT DEPOT, INC., ITS OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS (“RELEASEES”) FROM, AND HEREBY WAIVE ANY CLAIM FOR, ANY INJURY OR OTHER HARM THAT MAY OCCUR TO MY CHILD(REN) WHILE ATTENDING THE CAMP, OR ANY INJURY OR HARM THAT MAY OCCUR TO ME AS A RESULT OF THE INJURY OR HARM SUFFERED BY MY CHILD(REN) AT THE CAMP. I AGREE TO INDEMNIFY, DEFEND, AND HOLD HARMLESS THE RELEASEES AGAINST ANY AND ALL CLAIMS, SUITS, OR ACTIONS OF ANY KIND WHATSOEVER FOR LIABILITY, DAMAGES, COMPENSATION OR OTHERWISE BROUGHT BY ME OR ANYONE ON MY BEHALF OR ON BEHALF OF MY CHILD, INCLUDING ATTORNEYS’ FEES AND ANY RELATED COSTS. THIS RELEASE, INDEMNITY AND WAIVER SPECIFICALLY INCLUDES BUT IS NOT LIMITED TO (A) ANY CLAIM OF NEGLIGENCE OR NEGLIGENT SUPERVISION AGAINST RELEASEES; (B) ANY INJURY, HARM, OR DEATH THAT MAY OCCUR WHILE MY CHILD(REN) IS ENGAGED IN A CAMP ACTIVITY WHETHER CAUSED BY THEIR OWN NEGLIGENCE OR THE NEGLIGENCE OF A RELEASEE; AND (C) ANY INJURY OR HARM THAT MAY OCCUR WHILE MY CHILD(REN) IS OTHERWISE ON THE PROPERTY OF CAT DEPOT, INC. BEFORE, DURING, OR AFTER THE SCHEDULED CAMP HOURS FOR ANY REASON WHATSOEVER.

NOTICE TO THE MINOR CHILD’S

NATURAL GUARDIAN

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF CAT DEPOT, INC., ITS OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS USE REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD’S RIGHT AND YOUR RIGHT TO RECOVER FROM CAT DEPOT, INC., ITS OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND CAT DEPOT, INC., ITS OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS HAVE THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.

I hereby acknowledge and represent that I have been given the opportunity to review the Camp application, the medical information form and this permission form. I further represent that I have been given the opportunity to ask any questions concerning the information contained in the Cat Depot Acknowledgment and Agreement Form and this permission Form and I am satisfied with whatever response was given to any inquiry that I have made.

I hereby release and hold harmless Cat Depot, Inc, its officers, directors, employees, and agents from any responsibility or liability for any lost, stolen, or damaged personal property which my child(ren) brings to the Camp.
