Proof of vaccines being up-to-date is mandatory; current and past pets not being up-to-date on vaccines can be grounds for denying an application. We do contact your vet to confirm this.

If you purchase your heartworm preventatives online or someplace other than a vet, please upload the receipt or proof of purchase below. If you do not have this right now, please leave it blank and you can edit your application and provide your receipt at a later date.
