Facility Request Form
We welcome your interest in using our facilities. This form is required to be completed by anyone seeking to use the facilities of Central Baptist Church. (CBC Ministry Leaders should request use of our facilities for your ministry events through our REALM Facility Scheduling Group.) Please complete your details below and submit the form. SUBMITTAL OF THIS FORM DOES NOT GUARANTEE THAT YOUR REQUEST HAS BEEN APPROVED. You will be contacted once the data has been reviewed by staff. NOTE: Wedding Requests are for CBC Members only and must fill out a different form.

I understand that my selections in video/music will be required to meet the standards of Central Baptist Church and is subject to being edited/rejected if these standards are not met.

Use of our facilities may requires the following fees: Facility Use Cleaning Fee= $175 Media Fee= $100 minimum for Sanctuary booking. // $50 minimum for Gym booth booking.Additional fees may be required based on number of tech people needed for your event.
