CDDO of Butler County Award Nomination
Please use this form to make nominations for the Awards Luncheon on March 14th, 2025. Nominations need to be returned to the CDDO no later than March 5th, 2025. Nominations must include a brief narrative supporting the reasoning for your nomination of an individual.
1. Male Helping Hands Nomination:
A male client who is always willing to help their agency, staff, and other clients, regardless of their own disability.
2. Female Helping Hands Nomination:
A female client who is always willing to help their agency, staff, and other clients, regardless of their own disability.
3. Male Special Olympics Superstar Nomination:
A male client who actively participates in Special Olympics of Butler County and has shown outstanding dedication and time to their sports.
4. Female Special Olympics Superstar Nomination:
A female client who actively participates in Special Olympics of Butler County and has shown outstanding dedication and time to their sports.
5. Male Outstanding Acheivement Nomination:
A male client who has made exceptional strides towards their own independence and/or accomplished an incredible feat considering their disabilities.
6. Female Outstanding Acheivement Nomination:
A female client who has made exceptional strides towards their own independence and/or accomplished an incredible feat considering their disabilities.
7. Outstanding Volunteer to the Community:
An individual who has devoted their own time and efforts volunteering to the Butler County Community.
8. Outstanding Volunteer to I/DD Services:
An individual who has dedicated their own time, efforts, and/or resources to I/DD Services.
9. Outstanding Direct Care Staff:
A Direct Care Staff working in Butler County who has gone above and beyond to assist in meeting the needs of individuals in their care.
10. Outstanding Targeted Case Manager:
A Targeted Case Manager working in Butler County who has gone above and beyond to assist in meeting the needs of individuals on their caseload.
