I understand and agree that Cedar Bark Lodge is not liable for any injuries, illnesses, or accidents that may occur during boarding or grooming. Your pet’s safety is our TOP priority, however animals are animals and sometimes accidents happen.I hereby give permission for Deandra Camara/Cedar Bark Lodge to obtain Veterinary care if needed in my absence. I also agree to pay all costs related to such care.I acknowledge that my dog has no known contagious diseases and is up to date on vaccinations.I agree to release Cedar Bark Lodge from any liability related to my dog's health or behaviour during services. I understand that while all measures will be taken to keep my dog safe, and healthy; occasionally accidents do happen. I hold harmless Deandra Camara/Cedar Bark Lodge and agree to pay any/all charges incurred if vet care is sought or if my pet causes harm/damage to a person/pet/property.
I authorize Cedar Bark Lodge to perform grooming services as requested.I authorize Cedar Bark Lodge to board my dog and take necessary precautions for its safety, as they see fit. This may include things such as, leash walks in the yard, limited access to their kennel’s outdoor private space, limiting access to other animals etc.I understand that grooming and boarding may involve certain risks and I accept these risks.
I acknowledge that my requested boarding dates are NOT confirmed by submitting this form.I acknowledge that a meet and greet must take place before any boarding services will be provided.I understand that misrepresentation of my dog’s behaviour (ie. undisclosed aggression, illness etc.) may result in care being terminated early and my emergency contact being asked to pick up my dog.I understand that any excessive damage caused by my dog will incur charges for repair.By signing below I agree to be forthright in regards to my dog’s temperament, health and needs. Failure to disclose known behaviour requiring extra management, illness, or existing conditions/injury may result in my reservation being cancelled.
