    • I, *, (parent/guardian), provide the following medical information for* (full name of all campers) in connection with participation in Central Alabama Marines-Council’s Project Pathfinder Summer Camp.

      I hereby authorize Central Alabama Marines-Council, its contracted summer camp staff, and its partner organizations to act on my behalf, according to their best judgment, in any emergency requiring medical attention for my child(ren) during the Project Pathfinder Summer Camp. This authorization includes securing medical treatment, transportation, and related care when I cannot be reached in a timely manner.

      I further acknowledge and agree to waive and release Central Alabama Marines-Council, its staff, volunteers, and partners from any and all liability for injuries or illnesses that may occur as a result of participation in camp activities.

      To the best of my knowledge, my child(ren) have no physical or medical condition(s) that would be adversely affected by participation in the Project Pathfinder Summer Camp, except as disclosed in the medical information provided.

    • I    *   *   (parent/guardian), hereby give permission for my child(ren),*    to attend Project Pathfinder Summer Camp operated by Central Alabama Marines-Council and hosted at New Hope Cumberland Presbyterian Church, 5521 Double Oak Lane, Birmingham, AL 35242.

      Camp sessions will be held June 1–26 and July 6–24, between the hours of 8:00 AM and 2:30 PM.

      To enrich the camp experience, offsite field trips may be scheduled during the program. Parents/guardians will receive advance notice of all offsite activities, including details regarding location, transportation, supervision, and timing. Participation in any offsite trip will require a separate signed permission form, and only campers with completed authorization will be permitted to attend.

      I understand and agree to assume all financial responsibility for my child(ren) in the event of injury, as well as for any damage caused by my child(ren) to persons or property during participation in camp activities.

      I further acknowledge that Central Alabama Marines-Council and New Hope Cumberland Presbyterian Church are released from liability for injuries that may result from normal camp activities. In the event of an emergency involving my child(ren), I authorize camp staff and representatives to take reasonable and appropriate action to ensure their safety and well-being.

    • Media Release & Consent – Project Pathfinder Summer Camp 
    • I hereby grant permission for my child(ren) to be photographed, filmed, or recorded during participation in Project Pathfinder Summer Camp for the purpose of supporting the community impact mission of Central Alabama Marines-Council.

      I understand that photographs, video recordings, and other media may be used in promotional materials, social media, publications, presentations, or other outreach efforts related to Project Pathfinder and the organization’s nonprofit activities. I further acknowledge that any proceeds generated in connection with Project Pathfinder will directly support the nonprofit mission of Central Alabama Marines-Council, and that no participant will receive financial compensation for the use of such media.

      I also understand that, from time to time, news media, state and local agencies, community leaders, and educational partners may document or report on Project Pathfinder activities. This form serves as advance consent and legal release for the use of my child(ren)’s likeness in such circumstances.

    • Project Pathfinder's Summer Camp is a high-quality program designed to enhance the educational growth, character development, and personal enrichment of your child(ren). To ensure that all participants receive the full benefit of this experience, we require a strong commitment to regular attendance and engagement.

      This program is made possible through the generosity of state/community donors and partners supporting Central Alabama Marines-Council's community impact mission. Maintaining consistent attendance helps maximize the impact of these resources and ensures each camper fully benefits from this opportunity.

      Attendance Requirement: To remain eligible to participate free of charge, your child(ren) must attend at least 90% of all scheduled camp days. This requirement supports program continuity, academic progress, and responsible stewardship of donor-funded resources.

      I, the undersigned, acknowledge and agree to the following:

      • I understand Project Pathfinder has limited capacity available. If my child(ren) are accepted, I will be required to confirm my intent to enroll in the summer camp within five calendar days of notification.* 
      • At orientation night, or on the first day of camp, I will provide sufficient documentation verifying that a parent/guardian has served in the Active Component, Reserve Component, National Guard, or as a Veteran under the United States Department of War (or the former Department of Defense). *     
      • I attest that my child(ren) is(are) considered rising 1st through 8th grade students.*       
      • I understand that my child(ren) is(are) required to complete two Renaissance STAR Assessments (initial and final) as a requirement of enrollment in Project Pathfinder's summer camp.   *
      • I consent to my child(ren) participating in all programmed activities planned for Project Pathfinder's summer camp.   * 
      • I understand that normal camp hours are 8:00 AM – 2:30 PM, Monday through Friday, except during holidays or scheduled program adjustments. Campers may be dropped off no earlier than 7:45 AM and must be picked up promptly at 2:30 PM. I acknowledge that timely drop-off and pick-up are required to support proper supervision and program operations, and that repeated late pick-ups may result in additional fees. * 
      • I understand that a parent or guardian is responsible for providing transportation to and from camp each day.*
      • I understand that campers are required to bring a packed lunch each day unless otherwise communicated by camp staff for special events or scheduled activities. *        
      • I acknowledge that the host facility, New Hope Cumberland Presbyterian Church, is not a nut-free facility. While reasonable precautions may be taken, Central Alabama Marines-Council cannot guarantee an environment free from peanuts or other nut products. I agree to notify camp staff of any food allergies or dietary restrictions and to take appropriate precautions for my child(ren). *    
      • I understand that my child must be present for 90% of the programmed days to be eligible to attend the camp free of charge.  *        
      • I will commit to my child’s regular attendance and understand that failure to meet the attendance requirement may result in the revocation of the privilege to attend this no-cost summer camp.  *     
      • I will communicate any extenuating circumstances that may affect my child's attendance to camp staff, as soon as possible.  
      • I understand that all campers are expected to follow the rules and behavioral standards established by Project Pathfinder Summer Camp. If my child(ren) engages in conduct that is unsafe, disruptive, or repeatedly violates camp policies, I acknowledge that the camp reserves the right to take appropriate disciplinary action, including temporary suspension or immediate removal from the program. I further understand that removal due to behavioral concerns may occur at the discretion of camp leadership, and I agree to promptly arrange transportation for my child(ren) if such action becomes necessary. * 
      • I recognize and respect the time invested in planning this 7-week summer camp experience and the financial assistance provided by donors that enabled Project Pathfinder to occur this year.  * 
      • I understand the camp registration fee is $100/family to attend the 7-week program in June and July, or $50/family to attend the 4-week program in June only. *     
      • I understand that Project Pathfinder uses Procare Online for all camper check-in and check-out procedures. Any individual(s) authorized to drop off or pick up my child(ren) must be listed in the system and will be required to use their assigned unique PIN at each check-in and check-out. I acknowledge that only authorized individuals with a registered PIN will be permitted to sign a child in or out, and valid identification may be required upon request. *
