STATEMENT OF GUARANTOR: I am the guarantor whose name, signature and post assignment appears below who acted as the Guarantor of coop loan applied by the borrower whose name above, hereby assume all unpaid balance of said loan after the lapse of five (5) Months from the date of RELEASE and further authorized the Canteen to deduct the same from my salary until the amount shall have been fully paid.
