1. Absolve Chapel Place Sports Camp and CPPC of all responsibility with respect to any injury to my child during his/her participation in this program.
2. Give permission for both to attain medical treatment if required.
3. Give permission for my child to be fully involved in the program, and understand that my child may be sent home in the event that he/she does not abide by the rules of the camp.
4. Understand that it is my responsibility to pack a cold lunch for my child every day.
5. Understand that, to become fully registered, a submission of payment with the total registration fee must be made.
6. Understand that, Chapel Place Sports Camp has a zero tolerance policy regarding bullying of any kind and has committed itself to making sure all campers feel safe at all times while they are at camp. Bullying of any kind will not be tolerated and will be dealt with seriously.
7. Understand that, the Cancellation Policy will be applied if I do not cancel my registration more than one week before my child's first day of camp.
8. Understand that, a COVID-19 Screener will need to be administered weekly to staff and campers in order to allow entry to camp and to ensure safety among all participants.
