Party Booking Form

Birthday parties are held on Saturdays and Sundays and are supervised by a qualified ISAF cheer and dance coach. A non-refundable booking deposit of $100 is required to secure the day and time of your party. You may arrive 30 minutes prior to the start of your party to set up. You must follow the schedule of the party and will be expected to end your party at the agreed time. All food and drinks must stay in the reception area. Any adults on the floor must take off their shoes and are NOT permitted on any of the equipment. Balance of payment must be received prior to the party commencing. It is the responsibility of the hosting family to advise any medical details of children attending.

I further understand that medical/liability insurance is the full responsibility of the parent/guardian. I agree to provide the total number of guest prior to the party. I acknowledge that additional fees will be charged if I my party exceeds the scheduled time limit or if the number of guest change.
