Amazing Animals Play Kit Request Form

Please use this form to request your kit and sign-up for a contactless pickup on The Children's Playhouse porch. Kits are designed for kids aged 2 to 6 with enough materials for at least two children. One kit per family, please!

The Amazing Animals Kit has supplies and detailed instructions for 11 hands-on activities including Mini Animal Habitats with 4 animal figures, Wikki-Stix sculpture, Animal ear crowns, Hammering, Finger painting, Animal pancakes, and more.

We are gluten-free. Please substitute a gluten-free pancake mix. If the gluten-free option is not available, please omit the pancake mix. Please mail my kit. I will add an additional $10 for shipping to my donation at the end of this form and will include my address below.

Pick up Procedures

The kit will be labeled with your name and put on the Playhouse porch at the time you request below and will be left on the porch until the end of the day. We ask that only one person enter the porch area at a time. The Playhouse building and the playgrounds remain closed to the public, so please do not try to enter. For your protection, please wait 72 hours from the date the kit was sealed which is marked on the box.

by requesting a Playhouse STEAM Kit, do hereby release The Children’s Playhouse and its staff or volunteers from any and all liability for injury, illness, or any other condition of my child(ren) that may occur as a result of using The Children’s Playhouse Play Kits and waive my right to pursue legal action against The Children’s Playhouse or its staff or volunteers in connection with any such, injury, illness or condition.

Leave this kit unopened for 72 hours from the date the box was sealed. The Children’s Playhouse has followed current CDC recommendations and utilized reasonable mitigation strategies concerning COVID-19, so that the assembly and distribution of the Play Kits is as safe as possible. The Children’s Playhouse, however, cannot make any guarantees about potential exposure to COVID-19, with respect to supplies included in the Play Kits or staff/volunteers assembling such Play Kits, due to the highly contagious nature of the virus. Accordingly, The Children’s Playhouse recommends leaving each Play Kit unopened for seventy-two (72) hours from date of seal before use and it is MY responsibility as a parent to do so.
Remove activities that are not right for my child. The activities included in the Play Kit are designed for children of various ages and I understand that it is MY responsibility as a parent to remove activities that may be inappropriate because of age or other reason from the Play Kit before it is used by my child(ren).
Supervise my children while using these supplies. As a parent, I am responsible for monitoring the usage of the Play Kit by my child(ren).

Minimum Suggested Donation

Donations defray the materials costs of preparing the kits. Donations above $25 allow us to provide free kits to low-income families. Thanks to Mast General Store for assisting us with shipping at this reduced rate.

Please find my donation for materials below. If I requested shipping above, I have added an additional $10 to my donation to cover the shipping costs. I am requesting a free kit because I am a Playhouse member and am experiencing significantly reduced income at this time and/or I qualified for a scholarship membership prior to March 2020. I am requesting a free kit because I live in Watauga County and am experiencing significant financial hardship at this time. (Sorry, we can not provide free kits to nonmembers outside Watauga County.) I will leave a cash or check for the minimum donation when I pick up my kit. (Checks should be made to The Children's Playhouse. Please bring an envelope or use the ones provided at the donation box.)

We are currently experiencing significantly reduced income due the pandemic. Our family qualifies for Medicaid, WIC, and/or the North Carolina Food and Nutrition Services (EBT card). We have had to choose between paying for food and for other necessities (such as rent, medicine, utilities) in the last month. None of the above.
