Model Release Agreement

I, {q3_textbox1}, hereby grant {q4_textbox2} and their representatives, successors, and assigns the irrevocable and unrestricted right to use, reproduce, and publish photographs of me, taken on {q5_datetime3} at {q6_textbox4}, for any lawful purpose, including but not limited to advertising, trade, or any commercial or artistic use, in any medium now known or later developed.

I acknowledge that {q4_textbox2} owns the copyright in these photographs and I hereby waive any claims I may have based on any usage of the photographs or works derived therefrom, including but not limited to claims for invasion of privacy, right of publicity, or defamation.

I represent and warrant that I or my Parent/Guardian are at least 18 years of age, have the full legal capacity to execute this agreement, and have read and fully understand the contents of this release. This agreement shall be binding upon me, my heirs, legal representatives, and assigns.

I acknowledge that I may receive no compensation for this release and all rights granted hereunder, depending on the above selection made, and the amount mentioned is correct and will not change once this agreement is completed.
