I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against Memphis Baptist Church and the persons in charge.

I certify that my child is physically able to participate and has no medical condition that would prevent safe involvement. I understand it is my responsibility to inform the organization of any medical conditions or limitations.

I grant permission for my child to receive emergency medical treatment if necessary, and I accept responsibility for any costs related to such treatment.
