2025-2026 School Year Registration

Please complete the registration form for the upcoming school Fall 2025 - Spring 2026 School year.

Fall Registration Instructions:
1. Complete all pages and sections of this form and use the form to help you calculate payment.
2. Submit payment. Payments received by {hiddenEarly} to qualify for an Early Registration Fee waiver.
3. You will receive updates closer to the first day of school.
Please note:
- The school is designed for children K through 12. We accept children for our Kindergarten class who are aged five (5) and older when class starts in the Fall.
- The PTA Service deposit will be reimbursed after the family has volunteered a minimum service hours. Full year registrations is $130 deposit and will be refunded after a minimum of 12 volunteer hours.
- If you received a scholarship voucher for your child/children, please total them up and enter in the "Scholarship Discount" box in the "Family Adjustments" section near the end of this form.

Please note:
- The school is designed for children K through 12. We accept children for our Kindergarten class who are aged five (5) and older when the class starts.

Student Registration Information
Select the number of students you are registering in your family, then add information for each student. Please add as much information as possible including Chinese name (if available). If you don't know the class/grade level, you can select "unknown / TBD" and we will work you and the student to ensure proper placement.

If you have multiple students in your family, please enter your older/higher-grade-level students first. Sibling discount applies on the 3rd+ sibling, only on the tuition, and siblings with the lowest.

SFVCCA Membership fee $*
Lifetime members are families who have already paid for a separate $200 lifetime membership. They should select the Lifetime option.
Student Registration Fee*
Per Student. Waived if paid before the Early Registration deadline date. This is also waived for new students.
Student Event Fund*
Annual student/school activity fund. This supports school sponsored activities and events throughout the year. Calculated per student.
PTA Service Deposit $*
Annual PTA service deposit. Refundable after the family has volunteered the minimum qualifying volunteer hours. Per family.
Scholarship Discount
Enter your scholarship discount. Scholarships are valid for one year. Please contact clp.registration@sfvcca.org with any questions

The account name is "San Fernando Valley Chinese Cultural Association".I will mail a check made out to "SFVCCA/CLP" and mail to 8224 Yolanda Ave, Reseda, CA 91335. Please write your student's name in the memo/note.Pay Check or Cash in-person when school starts (The registration fee per student may apply if not received before the early registration deadline)

Policies & Authorizations
Refund Policy:
1. Full 100% refunds are available if requested before 8/23/2025
2. Partial 50% refunds are available if requested before 9/6/2025. This is also the last day to change / drop.
3. Please also note that each class will need to have a minimum of 3 students enrolled and the school reserves the right to cancel the class if the minimum registration is not met.

Authorization for Emergency Medical Care and Waiver of Claims:

I, hereby, give permission for the following student(s) to participate in SFVCCA language classes and cultural activities for the current semester/year. Should she/he become ill or injured during the school hours, she/he may receive necessary first aid and medical treatments. This authorization is given pursuant to Section 25.8 of the civil code of California State. I will not hold SFVCCA, its officers or teachers liable for medical and other expenses incurred in her/his care. I am hereby, waiving all claims against the Chinese Language Program or SFVCCA for injury, accidents, illness or death occurring during the program hours.
