  • I understand fully that this personal training program may include exercises to build the cardiorespiratory system (heart and lungs), the musculoskeletal system (which involves muscular endurance, strength and overall flexibility), and to improve body composition (increasing muscle and bone and decreasing body fat).

    Exercise includes aerobic activities such as walking, running, bicycle riding, rowing machine, group aerobics, swimming and other aerobic activities, weightlifting using dumbbells, machines and other equipment to improve muscular strength and endurance, as well as flexibility exercises to improve joint range of motion.

    Potential Risks:

    I understand that the reaction of the heart, lung, blood vessels as well as other systems to exercise cannot always be predicted with accuracy. I know there is a risk of certain abnormal changes occurring during the following exercise which includes abnormalities of blood pressure or heart attacks as well as other side effects. Use of weightlifting equipment and engaging in heavy body calisthenics may lead to musculoskeletal strains, pain an injury if adequate warm-up, gradual progression, and safety procedures are not consistently followed.

    I understand that Ebony Robinson and/or her staff shall not be liable for any
    damage arising from personal injuries sustained by the client while and during and/or from a personal training program does so at my own risk. I assume full responsibilities for any injuries or damages which may occur during and/or after training.

    I hereby fully and forever release and discharge Ebony Robinson and/or her staff from all claims, demands, damages, rights of action, present and future therein.


    I understand and warrant, release and agree that I am in good physical condition and that I have no disability, impairment or ailment preventing me from engaging in active or passive exercise that will be detrimental to heart, safety, or comfort, or physical condition if I engage or participate. (Other than the those listed on the pre-exercise questionnaire.)


    I state that I have had a recent physical checkup and have my personal physician’s permission to engage I aerobic and/or anaerobic conditioning.

    Potential Benefits:


    I understand that a program of regular exercise for the heart, lungs, muscles and joints has many benefits associated with it. These may include a decrease in body fat, improvement in blood fats and blood pressure, improvement in physiological function and decrease in heart disease.


    I have read the foregoing information and understand it. Any questions, which may have occurred, have been answered to my satisfaction.

  • Session Cancellation Policy: Clients maintain the right to cancel payment at any time in advance of the billing date for thenext month’s session. Cancellations must be done in writing (either by posted letter to the company or by emailing to the
    following email address: coachebony@eclassfitclub.com) and the letter or email must be received before the new billing cycle.


    If you need to cancel an appointment, please notify our office at least 24 hours in advance of the appointment date to reschedule that session or class.


    Termination Policy: Termination of the agreement may occur at the discretion of the coach when an Impasse is reached with the client. The goal is to better your fitness, so all efforts will be made towards that end. Examples of terminable offenses are lack of payment, consecutively being late, disruptive behavior (to coach and class), poor hygiene, etc.


    Fees: The fee for the coaching sessions will be charged on a monthly/bi-weekly basis in advance of that month’s session on a recurring cycle.


    Services: The client will be provided with monthly fitness coaching services: customized meal plans, nutritional tips and advice, workout tips and advice, email newsletters, and Access to Facebook VIP group.


    Confidentiality: Anything that is said or revealed in the sessions between coach and client is privileged information and will not be disclosed to any outside party. The exception is if the information revealed includes disclosure of illegal, unethical or criminal activities. The methods employed by E Class Fit Club do not guarantee results.


    They are guidelines that the client must follow through on in order to see any type of change. The client agrees, therefore, to hold the coach free of any liability and responsibility for adverse reactions as a result of advice given in the coaching sessions.
