I give my dancer permission to participate in class and performances with Colossal Dance Studio (Colossal). I understand that as a physical activity, there is possibility for injury/physical complications and do not hold the above company or it's employees responsible for any injuries or complications my dancer may endure. I understand that should anything happen, my dancer will recieve the best care possible and I will be notified as soon as possible. I understand that the protection of my personal property being brought into the studio is under the responsibility of my dancer and should anything entering the studio get lost or stolen, it does not fall on the responsibility of Colossal.
