Lay Committee Assessment

Directions: The lay committee is to share their reflections on the following five questions, discuss their reflections with the student, and share the completed form with the supervisor before the chair/convener signs and submits on behalf of the whole committee to the Director of Contextual Education. The committee is to provide a copy of the final assessment to the student who will submit the signed document to the director of contextual education after they have reviewed it.

The assessment is due no later than the Friday of week 10 the quarter it is being submitted.

Learning Goals: Please list the student learning goals as they are articulated in the Learning Covenant submitted at the beginning of the placement.*

Learning Goals: Toward which of the goals in the student’s learning covenant did you see the most progress thus far? What activities, actions, or behaviors did you observe that contributed to this progress?*

Strengths and Growth Areas: What strengths in ministry have you observed in the student? What areas of continued growth will benefit the student in future ministry?*

Student/Committee Relationship: How has the student received and responded to feedback and support from the lay committee? How has the student offered feedback to the committee?*

Overall Assessment: What is your overall assessment of the student’s placement experience? Are there any other reflections not covered above that might help the student?*

Once the lay committee chair and student feel the review is accurate, specific, and thorough, the overall review should be signed below by both parties before it is submitted.
