Supervisor Assessment

Quarter*

Directions: The student and the supervisor conduct the review together and then submit one assessment that is agreed upon and signed by both. The student and the supervisor should review and discuss their thoughts during a supervisory session. If the student or supervisor wish to make changes as a result of the discussion, those changes should be made and discussed together.

The assessment is due no later than the Friday of week 10 the quarter it is being submitted.

Student Learning Goals: Please list the student learning goals from the Learning Covenant submitted at the beginning of the placement.*

Learning Covenant Assessment: For each goal in the Learning Covenant, identify the ways in which the student has made progress toward the goal. Be specific in identifying activities and experiences that indicate progress. In what ways can or should the student continue to develop toward this goal? *

Overall Growth Assessment: Assess the student’s overall growth toward effective ministry, vocation, and identity so far. Consider the student’s relationship with authority (his/her/their own and others), the student’s effectiveness as a leader, the ways in which the student’s ministry was received, and the ways in which this specific placement has contributed to this growth. *

Supervisory Relationship Assessment: Describe some of the issues that have been discussed in supervisory meetings and in theological reflection. Describe the effectiveness of communication and feedback in the supervisory relationship. What are the strengths of the relationship and what are areas that have been challenging?*

Other Assessment: In addition to the Learning Covenant goals, what other activities or issues have had a meaningful impact on the experience? How should these be developed/addressed for the student’s future in ministry? Provide any final remarks or recommendations regarding the student, the supervisor, or the internship site.*

How well did the student address their goal(s) as stated in their placement site agreement? What specific activities stand out as part of addressing the goal(s)? If specific goals were not well addressed, what factors contributed to that result?*

What areas of ministry leadership stand out as opportunities for further development for the student?*

What areas of strength did the student exhibit in their time at the placement site, whether these strengths were addressed in the semester learning goals or not?*

What were the topics that the student covered in their theological reflections? In what ways did the student engage the supervisor in the process of reflecting theologically on these areas of ministry?*

Please provide an overall assessment of the student’s time at the placement site.*

Once both parties feel the review is accurate, specific, and thorough, the overall review should be signed below by both the student and the supervisor before it is submitted.
