    INITIAL CORE SERVICE PROVIDER APPLICATION

    ProfileGorilla Subscription: Please be advised that your company must subscribe to ProfileGorilla PreQual+ (our compliance vendor partner), and complete all required fields on the ProfileGorilla platform. After you submit this application to CORE Group, please email the ProfileGorilla team at coregroup@profilegorilla.com to get started. If you are already signed up for ProfileGorilla PreQual+, you can simply fill out the initial application below and contact the ProfileGorilla team to share your credentials that satisfy all of our program requirements. If your company has an existing ProfileGorilla PreQual+ Subscription, there will be no extra charge if CORE is your second active client. If you are already active with 2+ clients, there is a $35 one-time set-up fee. If you are a New ProfileGorilla PreQual+ Subscriber, there is a $65 one-time setup fee + $25/month (or $275/year fee) for the service of 1-2 locations, which provides access to many other TPA/Managed Repair programs. If you have 3-8 locations, there is a one-time $70 set-up fee, with a $99/month charge ($1,100/year). 9+ locations will have an $85 set-up fee and an annual cost of $1,540/year.

    One-Time Application Fee: You will be required to enter payment info for a one-time fee of $599 before submitting this application. Fee will only be refunded if you do not meet our initial application requirements. If you have any questions about this one-time fee, please email joincore@gowithcore.com.

    Estimate Requirements: Please Note: Here are some key requirements regarding our procedures for handling insurance claims.
    - Estimating in Xactimate: This is mandatory when working with PURE Insurance, subject to approval as a vendor by PURE.
    - Estimating in Mobile Claims (Symbility): This is mandatory for our other carriers. (You can utilize CORE's Remote Estimating Desk)
    - Digital Dry Logs: Please ensure that Digital Dry Logs are completed for all mitigation processes.
    These requirements are crucial to streamline our operations and maintain compliance with each carrier's guidelines. If you have any questions or need further clarification on these processes, please do not hesitate to reach out.

    CORE Group uses ProfileGorilla to on-board its Service Providers and maintain your credentialing and compliance for you, and it is required that you become a subscriber of ProfileGorilla's PreQual+ Service if you are not already.

    -Existing ProfileGorilla PreQual+ Subscribers: NO CHARGE
    -New ProfileGorilla PreQual+ Subscribers: $50 one-time set up fee ($15 CORE Perks Discount included) + $25/month or $275/year subscription plan ($25 CORE Perks Discount included)

    Additional Benefits to subscribing to ProfileGorilla PreQual+
    -ProfileGorilla can help connect contractors to new Client Programs or Managed Repair/TPA Programs who also use ProfileGorilla for compliance at no additional cost.
    -ProfileGorilla Team deals with all insurance compliance issues directly with contractor's Agent, including renewals.
    -If your organization orders background checks through ProfileGorilla, you’ll receive bulk buying power discounts, plus automation around ordering and report uploads (removes all the manual steps). However, your company can utilize any background check provider.
    -ProfileGorilla team pulls down State/Business License renewals directly from the License Bureau websites, so you don’t have to manually upload them. Lead/EPA (and soon IICRC) certificates can also be obtained by ProfileGorilla team.

If you are already signed up for ProfileGorilla PreQual+, you can simply fill out this initial application and contact the ProfileGorilla team to share your credentials that satisfy all of our program requirements on the ProfileGorilla platform.

    You can purchase discounted background checks through the ProfileGorilla platform (our compliance vendor partner), through ScreeningOne by contacting 
    Trish Gawlowski at 602-490-0355, trish.gawlowski@screeningone.com, or through The Background Check Company and Bryce Froberg at bryce@tbccomp.com or (206) 973-9312. You may also use any screening provider you wish, as long as it includes all of the requirements found below. Please submit/upload all background checks via the ProfileGorilla platform.

    You agree and understand as an CORE service provider that background checks are submitted every three years.

    Required Checks:
    -Social Security Number Trace, including 7-year address history and any associated alias or maiden names (AKAs)
    -County and Federal Criminal Record Search - 7 Years
    -National Criminal Database
    -Government Watch List
    -A national crosscheck of all sex offender databases and registries, as well as, state sex offender registries that encompass all counties in each state

    Ensure Coverage includes:
    -Primary & Non-Contributory Coverage
    -Premises & Operations Liability
    -Ongoing and Completed Operations Coverage for Additional Insured
    -Products and Completed Operations Liability
    -Contractual Liability with no employee exclusion
    No exclusions/restrictions are allowed for:
    -Claims from additions or remodeling projects
    -Residential or habitation
    -Coverage for work performed by subcontractors

    Auto Liability: $1,000,000 Combined Single Limit of liability per occurrence
    -Must include coverage by endorsement for all vehicles owned, leased, rented, hired, or borrowed.
    -If you do NOT own any Company Vehicles “HIRED AUTOS” and “NON-OWNED AUTOS” is still Required and can also be obtained as an Endorsement on your General Liability Policy or Unlimited Non-Owned Car Coverage (UNOC).

    Workers Compensation & Employers Liability: Employer’s Liability limits of $500,000
    -Workers’ Compensation is required per State statutory minimum limits.
    -If you do NOT carry Workers Comp and are NOT Required to carry it by State Statutes, a Workers Comp Attestation will be provided that must be signed and returned. In some States, your official State Workers Comp Exemption Certificate may also be required.


    PROPERTY RESTORATION SERVICE PROVIDER REQUIREMENTS FOR GC / MITIGATION / BIOHAZARD / REMEDIATION
