ASSUMPTION OF RISK & RELEASE OF LIABILITY
I, the undersigned, understand and acknowledge that participation in Core Ringette activities involves inherent risks, including but not limited to physical injury, falls, collisions, and other unforeseen incidents. I voluntarily assume all risks associated with my/my child’s participation.I hereby release, discharge, and hold harmless Core Ringette, its owners, instructors, staff, and affiliates from any and all claims, liabilities, damages, or expenses arising from my/my child’s participation, including those resulting from negligence. I confirm that my child/I am in good health and physically capable of participating in ringette training.
MEDICAL AUTHORIZATION
In the event of injury or illness, I authorize Core Ringette staff to administer first aid and seek emergency medical treatment if necessary. I accept full financial responsibility for any medical care provided
PHOTO/VIDEO RELEASE
I grant Core Ringette permission to capture and use photographs and videos of me/my child during training sessions for promotional, social media, and educational purposes. I understand that no compensation will be provided for such use. If I wish to opt out, I will notify Core Ringette in writing
ACKNOWLEDGEMENT & AGREEMENT
I have read and understood this waiver, and I agree to its terms voluntarily. I understand that by signing, I waive certain legal rights, including the right to sue.
