1. ASSUMPTION OF RISK. Participant understands and acknowledges that basketball training is a physical activity that involves inherent risks, including but not limited to: Falls, collisions, overexertion, muscle strains, sprains, fractures, head injuries, serious bodily injury or death. Participant voluntarily assumes all risks, known and unknown, associated with participation in training sessions, workouts, scrimmages, and related activities. 2. RELEASE AND WAIVER OF LIABILITY. Participant hereby releases, waives, and discharges the Trainer, business entity, owners, employees, contractors, volunteers, facility owners, and affiliates from any and all claims, demands, damages, injuries, losses, or causes of action arising out of or related to participation, including those caused by negligence, to the fullest extent permitted by law. 3. INDEMNIFICATION. Participant agrees to indemnify and hold harmless the released parties from any claims, damages, costs, or expenses (including attorney’s fees) arising from Participant’s actions, participation, or breach of this Agreement. 4. MEDICAL CONSENT. Participant certifies that they are physically and medically able to participate. Participant authorizes Trainer to obtain emergency medical treatment if necessary and agrees to be responsible for all related costs. 5. PHOTO, VIDEO, AND MEDIA CONSENT (VERY IMPORTANT) Participant grants full permission to Trainer to: Photograph, video record, and otherwise capture Participant’s image, likeness, voice, name, and performance. Use such media for marketing, promotional, instructional, commercial, and social media purposes, including but not limited to Instagram, TikTok, YouTube, websites, advertisements, and training materials. Participant waives any right to compensation, ownership, inspection, or approval of the media content and understands that content may be edited, published, or distributed at Trainer’s discretion. This consent is perpetual, irrevocable, and worldwide. 6. NO EXPECTATION OF PRIVACY. Participant understands that training sessions may be recorded in open gym or group settings and waives any expectation of privacy while participating. 7. SEVERABILITY If any portion of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect. 8. GOVERNING LAW. This Agreement shall be governed by and interpreted according to the laws of the State of California. 9. ACKNOWLEDGMENT AND SIGNATURE. Participant acknowledges that they have read this Agreement in full, understand its contents, and sign it voluntarily, intending to be legally bound.
