Insurance Company to pay Stan's

Body Shop directly for repairs to this vehicle:

I hereby authorize the repair work to be done along with the necessary material, and hereby grant you/your employees' permission to operate the vehicle herein described on streets, highways, or elsewhere for the purpose of testing/inspecting. An express mechanics lien is hereby acknowledged on the above vehicle to secure the amounts of parts.

Stan’s Body Shop is not responsible for the availability of parts, or delays in part shipments beyond their control. Once we order parts, if you cancel your appointment, a 20% restocking fee will be charged.

I also understand and approve of the use of aftermarket/recycled (LKQ) and/or reconditioned parts as per my insurance company guidelines.

If the vehicle is returned to the customer before authorized repairs are performed, a diagnostic fee, handling charge, reassembly labor, storage and 20% parts restocking fee will be due prior to the vehicle being released.

 

I do hereby appoint Stan's Body Shop to accept on my behalf any, and all checks, drafts, or bills ofchange, and endorse all such checks, drafts, or bills of exchange for deposit to their business bank account for credit on my account for repairs on my vehicle which has been released and accepted.I hereby agree to notify Stan's immediately, and I agree to deliver any checks to the repair facility within 48 hours of receipt of any insurance check.
