RELEASE AND WAIVER OF LIABILITY AGREEMENT

I, (hereafter known as the “Participant”), acknowledge that I voluntarily applied to participate in the Escape Room game simulation (hereafter known as the “Game Simulation”) at Crypto Escape Rooms Inc. (hereafter known as “Crypto Escape Rooms”).

I acknowledge that:

By signing this form I will be held responsible for any and all participants in my party.
The Participant may be placed in a small, enclosed area with limited lighting.
Crypto Escape Rooms is not responsible for the Participant in case of any anxiety or similar responses associated with claustrophobia or other similar disorders.
No coercive or excessive physical force should be used to participate in the Game Simulation.
Crypto Escape Rooms is not responsible for any bodily harm or injuries incurred from participating in the Game Simulation.
Crypto Escape Rooms is not responsible for any damage or loss of personal property of the Participant.

As consideration of being permitted to participate in the Game Simulation by Crypto Escape Rooms, I hereby attest that I and the rest of my party am in good health and physically able to participate in the Game Simulation, and on behalf of myself and my party I forever release Crypto Escape Rooms and their respective directors, officers, employees, volunteers, agents, contractors, and representatives (hereafter collectively known as the “Releasees”) from any and all actions, claims, or demands that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives now have, or may have in the future, for injury, death, or property damage, related to (i) my participation in these activities, (ii) the negligence or other acts, whether directly connected to these activities or not, and however caused, by any Releasee, or (iii) the condition of the premises where these activities occur, whether or not I am then participating in the activities. I also agree that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives will not make a claim against, sue, or attach the property of any Releasee in connection with any of the matters covered by the foregoing release.

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN MYSELF AND CRYPTO ESCAPE ROOMS, AND SIGN IT OF MY OWN FREE WILL.
