  • ROAD AND BRIDGE APPLICANTS:

    All driver applicants must provide information on all employers during the preceding three (3) years, including full address. Commercial motor vehicle driver applicants must further provide an additional seven (7) years' information those employers for whom the applicant operated commercial vehicles. This includes vehicles having GVWR over 26,001 lbs or more, vehicles designed to move 16 passengers or more, or any size vehicle used to transport hazardous materials in a quantity requiring placarding.
