Travel Protection/Insurance Waiver
Unexpected circumstances can arise. A sudden health emergency for you or an immediate family member just before or during your trip could spoil your vacation. If you had to cancel or your trip were interrupted, would your money be refundable? In most cases, the answer is "no". Many events, natural disasters (like hurricanes, earthquakes, volcano eruption), COVID-19, and events such as political unrest can affect your travel. Also, missed connections due to airline maintenance, re-routing or even missing cruise ship departures can lead to losing out on your investment, even if these acts are out of your hands.
It is also important to note that medical attention is also not guaranteed to be covered while traveling, especially outside the country. It is the travelers' respoinsibility to investigate and review your cancellation policies from your suppliers as well as your private medical coverage.
Custom Memories Travel has a responsibility to make clients aware of the possibilities during travel. It is required that a travel protection waiver such as this be signed by the traveler if the traveler declines any type of travel protection/insurance. Custom Memories Travel would be glad to revisit the addition of this protection for your trip if you desire.

I, the representative of the traveling party, do not wish to purchase travel protection and cancellation insurance for this vacation and acknowledge that I have been offered coverage and have declined.
I understand that I am liable for any cancellation penalties and out-of-pocket expenses incurred should my travel plans change.
I understand that whether travel protection is purchased or not, COVID-19 rules and regulations are always changing and it is the traveler's responsibility to meet those travel requirements. The travel agent is only able to provide guidance and does not have details on all regulations in all countries at all times.
I understand that my travel agent can help suggest alternative arrangements should and emergency occur. However, I also understand that I will need to be available for the actual booking process and will have to reimburse the agent for any fees involved that they may incur on my behalf (on top of penalties and cancellation fees). This will include the $25 per person ticketing fee for each leg of domestic flights and $50 per person for each leg of international flights and may include other fees.
I understand that I am solely responsible for any medical services delivered whether covered by my own health policy or not.
