This form must be on file for your child’s participation in any event sponsored by MET Summer Academy, a program in partnership with Choral Arts Link, Inc. and the National Museum of African American Music.
MET Summer Academy decorum expectations are in compliance with Metropolitan Nashville Public School standards of conduct. Guidelines set by Metropolitan Nashville Public School Board of Education for safety of students, teachers and staff apply during MET Summer Academy.
I am responsible for any and all transportation for my child to and from MET Summer Academy. I will daily sign in and sign out my child from the camp facility.
I acknowledge that the teachers, staff and guest artists involved in MET Summer Academy are providing their time and expertise to enhance participants' performance artistry, musical education and instruction. I, in turn, will make every effort to deliver and pick-up my child in a timely manner for MET Summer Academy. If my pickup is delayed 15 minutes after session closure I will alert the CAMP Director, 615-260-5278. If my pick up is delayed twice (2 times), my child will be removed from MET Summer Academy without a refund.
I will discuss with my child that it is an honor to participate in MET Summer Academy and that he/she represents his/her family, school, community and city. This is a responsibility that students and teachers will take very seriously. Decorum and cooperation from all participants (singers and adults) should be exemplary at all times.

Safety guidelines as set by Metropolitan Nashville Public Schools (MNPS) Board of Education apply during MET Summer Academy for participants, teachers, staff, volunteers and special guest. Each staff member (Artistic Associates, Director Assistants) and volunteers are responsible for the safe keeping of each participant and will follow the guidelines set by Metropolitan Nashville Public Schools and the Tennessee State University Security Compliance Regulations.
Each participant is responsible for cooperating with teachers, staff, volunteers, guest artists and fellow participants for best experiences for everyone.
If at any time a participant is uncooperative and a resolution cannot be determined through conferencing with the staff/camp director and the student’s parents/guardians, said participant may be asked to withdraw from the remainder of any or all scheduled activities (effective immediately).
I have read and understand the student decorum expectations. I will abide by them for MET Summer Academy.
I realize that in the event my child is injured during MET Summer Acadmey and therefore, in order that my child may participate, agree as follows: I agree that neither MET Summer Academy, Choral Arts Link, Inc., NMAAM and its Boards of Directors, nor any contributor or sponsor of the event, their members, employees, agents, sponsors, officers, directors, or officials will be held responsible for any injuries that my child might incur in conjunction with the above described events and/or during transport to or from MET Summer Academy.
I release and forever discharge any contributor, sponsor, or partner of the event, their members, employees, agents, sponsors, officers, directors, or officials from any injury that our child might incur, including death, and from any and all claims, demands, damages, actions, and causes of action whatever kind and nature which might occur. As further consideration for allowing my child to participate in this event, I further agree to indemnify and hold harmless any contributor, sponsor, or partner of the event, their members, employees, agents, sponsors, officers, directors, or officials from any claims or actions and costs, brought by us or any other party on our behalf on account of any claims, damages, actions, and causes of action of whatever kind and nature arising out of the aforesaid event.

MET Summer Academy, Choral Arts Link, Inc. and NMAAM have my permission to photograph/video my child with still or motion picture or both and/or have his/her voice reproduced.
I agree to the use of such photographs/video/voice reproductions and internet, including but not limited to (website and/or internet pages/hard copy products) for publicity and promotional purpose by MET Summer Academy and its partners, and I hereby waive all claims for further compensation for such use or for damages. I waive any right that I may have to inspect or approve the finished product or the copy that may be used in connection with the use of such photographs/videos/voice reproductions.
I also agree to the requirement of a photo ID to be made of my child on campus to meet the security regulations for summer camps at Tennessee State University.

Requested information is gathered so we may offer the safest environment or response to a singer’s needs. It is reviewed in the event MET Summer Academy faculty and/or staff need to attend to your child's/youth’s health. Food allergy information is requested for events where snacks/meals are provided.
Because we continue to grow each year, we are asking for parent, civic, and community volunteers to help camp coordinators manage the daily camp tasks. MET Summer Academy Registration Relief (RR) is an initiative of Lynetra Dunn, an alumni MET parent & former volunteer coordinator. Registration Relief allows families who are qualified for free or reduced lunch to receive a fee reduction. (A fee reduction does not mean your singer attends for free.) If your child receives free/reduced lunch, based on the United States Federal 2016/2017 qualifications, then your child will qualify for reduced registration fees for grades 2 - 4 & 9 – 12 only.
June 12th-June 30th 2017:
Grades 2-4 RR Fee $175
Grades 5-12 RR Fee $275
*No Registration Relief for Middle Schools (Grades 5-8)
Proof must be one (1) of the following and must be presented when your singer is registered.
(You may register before or on the first day of camp, June 12th)
1. Local school district’s official Statement of Eligibility received each year by qualified families. A copy of the Statement of Eligibility must be received on or before June 5, 2017. 2. A typed statement on your local school’s letterhead expressing knowledge of family's free/reduced lunch eligibility.

One payment may be made for multiple siblings, but fill out a separate registration form for each child. NO additional reductions in fees for multiple siblings.

MAILED PAYMENTS:Basic and Registration Relief onsite payments are accepted no later than June 5th. ONLY money orders will be accepted. Mail to: 460 10th Circle North, Nashville, TN 37203 *We will only take money orders from Walmart, Kroger or Wester Union. No checks will be accepted onsite.
REGISTRATION RELIEF PAYMENTS:After we receive your information, we will contact you. When you have received confirmation that you are eligible for reduced camp fees, you will then be able to make payments online or onsite.
