當您希望更改銀行詳細資料時，請填寫此表格。您可以直接在手機、平板電腦或電腦上輕鬆填寫此表格，無需列印。

填写表格应该需要10 -15分钟。

在您完成此表后，请检查所填写的资料，之后请在最下方按提交键，就完成了手续！

僅附上相關文件並與填妥的表格一起寄回。只有當您填寫表格、接受條款和條件並提交所有必需的證明文件後，我們才能更改您的銀行詳細資料。如果在當月 20 日之後請求更改銀行詳細信息，新的銀行詳細資料仍將從您的舊銀行帳戶中扣除。

Please fill in this form when you wish to change your bank details. You can easily fill out this form directly on your phone, tablet or computer without printing. Filling out the form should take 10 -15 minutes.

After you complete this form, please check the information you filled in, and then press the submit button at the bottom to complete the procedure!

We can only change your banking details if you have completed the form, and submitted all the required supporting documents. If the change bank details is requested after 20th of the Month the new bank details will still deduct on your old bank account.

 Policyholder own bank account
a) 帳戶證明（不超過三個月的銀行對帳單或銀行信函）Proof of account (bank statement or bank letter not older than three months)
b) 投保人身分證、護照影本 A copy of your ID, passport 

  第三方帳戶 Third-party account
a) 第三者的帳戶證明（不超過三個月的銀行對帳單或銀行信函）3rd party's Proof of the account (bank statement or bank letter not older than three months)
b) 第三方（銀行帳戶持有人）身分證、護照影本 A copy of the third party’s (bank account holder) ID, passport 
c) 投保人身分證、護照影本 A copy of the /policy holder's ID, passport or driver's license

公司帳號 Company account

a) 公司帳戶證明（不超過3個月的銀行對帳單或銀行信函）Proof of company account (bank statement or bank letter not older than 3 months)
b) 各簽字人或公司授權代表人的身分證及護照影本。Copy of ID card and passport of each signatory or person authorized to sign on behalf of the company.
c) 投保人身分證、護照影本 A copy of the policyholder’s identity card, passport
d) 授權書，包括所有授權人員的詳細資訊以及授權適用的保單或會員資格的詳細資訊。授權書必須簽名並註明日期，並授權公司帳戶根據授權書中指定的保單或會員詳細資料扣除費用Authorization letter including details of all authorized persons and details of the policy or membership to which the authorization applies. The authorization letter must be signed and dated and authorizes the corporate account to be debited against the policy or membership details specified in the authorization letter
e) 公司註冊證影本 Copy of company registration certificate
