How and where to remit payment?
Girl Scouts – Dakota Horizons will invoice you for the cost of the insurance based off of the information provided by you in this form.
What happens after I submit payment?
Your form will automatically be sent to the Girl Experience Manager (ge@gsdakotahorizons.org) who will then purchase the appropriate insurance. The Girl Experience Manager will send you a confirmation email that your insurance has been purchased, and what the insurance covers.
Submitting this form does not automatically mean that coverage has been purchased. Please submit 2-3 weeks prior to your event/trip. You will get a confirmation email when the insurance has been purchased.
