     Plan 1 Basic Accident Only Included with Annual Membership For all member and nonmember participants of Girl Scout sponsored/supervised events and travel within the U.S. territory.
     Plan 3P Primary Accident & Sickness $0.70 per day For events lasting longer than those covered by plan 1 when use of the family health plan is not possible.
     Plan 3E Excess Accident & Sickness  $.29 per day For events lasting longer than those covered by Plan 1. Coordinates with any family health plan.
     Plan 3PI Primary Accident & Sickness & Travel Assist  $1.17 per day Events outside USA

    *$5 minimum

    How and where to remit payment?
    Girl Scouts – Dakota Horizons will invoice you for the cost of the insurance based off of the information provided by you in this form.

    What happens after I submit payment? 
    Your form will automatically be sent to the Girl Experience Manager (ge@gsdakotahorizons.org) who will then purchase the appropriate insurance.  The Girl Experience Manager will send you a confirmation email that your insurance has been purchased, and what the insurance covers. 

    Submitting this form does not automatically mean that coverage has been purchased.  Please submit 2-3 weeks prior to your event/trip.  You will get a confirmation email when the insurance has been purchased.
